Emmet Place Family Practice is accepting new patients. All new patients will need to complete our Patient Registration form. If you are transferring from an existing clinic, you will also need to complete a Transfer of Medical Record Request Form.

If you are a medical Card or GP Visit cardholder, and would like to transfer to our clinic, you will need to complete HSE Change of Doctor Form.

These forms must be submitted to the Practice Manager before attending the surgery.
